1. Under the Create menu, Click "Question set"
2. Click "Create question set"
3. Add a title for your Question Set
4. Enter the question
5. Select the type of Question
Dropdown, short answer, multi-select etc.
6. Enter the options for the answers
7. Select if this is a required question or not
if the question is required, they will not be able to proceed to the next step in the registration process unless the question has been answered
8. Click "Save question set"
9. Click "Done"
